Keep a practice assumptions register
Record what you currently believe, what decision depends on it, what evidence could change it, who owns the follow-up, and when you will revisit it. Never blend assumptions with verified facts.
Understand the work
Before the checklist
What this work is really for
Every early plan contains assumptions. The risk begins when an assumption loses its label and later choices treat it as settled.
If you are new to ownership
Capture statements that begin with probably, usually, I think, or the vendor said. You are not expected to know everything; you are expected to keep uncertainty visible.
If you already run a practice
Use the register to find old beliefs that still shape budgets, staffing, software, or timelines after the practice changed.

Immediate actions
Get oriented before doing the work.
- Keep facts, assumptions, questions, and decisions visibly different.
- Connect each consequential assumption to the decision it could change.
- Preserve a dated history when evidence changes the plan.
Make sure this fits
Use this while describing, launching, or revising a practice and whenever several decisions depend on incomplete information.
Pause when
- The register is being treated as evidence, approval, professional judgment, or a risk assessment.
Gather before you begin
- A list of active planning beliefs and open questions
- A safe local workspace without client or sensitive workforce information
Expected output
- A dated register of assumptions, dependent decisions, evidence needs, owners, and review triggers
A labeled assumption can be researched; an unlabeled assumption can quietly influence contracts, budgets, launch promises, and regulated decisions.

Guided process
Work through it, one decision at a time.
- 01
Capture and label assumptions
Pause or get help whenConfidence is not approval; regulated conclusions still require authoritative evidence and qualified review.
- 02
Assign evidence and review
Pause or get help whenPrioritize assumptions affecting rights, safety, privacy, employment, billing, records, accessibility, security, contracts, or opening timing.
- 03
Review and preserve changes
Pause or get help whenReopen assumptions when profession, state, service, owner, setting, payer model, workforce, technology, or information use changes.
Finish well
Adapt, record, review
Leave a useful trail for the next person.
If your situation is different
- A solo owner can use a simple table; a team can add decision owners and reviewer handoffs without adding sensitive data.
What good looks like
- Facts and assumptions are visibly distinct
- Every consequential assumption names a dependent decision
- Evidence and reviewer needs have owners
- Changed assumptions retain history
- No PHI, credentials, or sensitive identifiers are stored

Common mistakes
- Treating confidence as proof, using vendor or peer statements as authority, or deleting an assumption after it changes.
